| Activity Terms and Conditions 1. All activities are at the users own risk.
Maui Dive Shop is acting as a ticket agent only and has no responsibility for any damage or injury
resulting from any activity.
2. When booking activities through Maui Dive Shop, no credit card will be charged for any activities until
booking is confirmed with activity company.
3. Cancellation policies vary based on individual activity. Please be sure you are aware of the
cancellation policy for each individual activity.
4. Refunds due to illness or extenuating circumstances, within the cancellation penalty period, will be at
the discretion of the individual activity company.
5. Medical Release Forms are required for all instructional dive classes.
6. Dive Certification Cards must be presented at the time of check-in for your dives. If you do not have
your Certification Card you will not be permitted to dive.
Product Return Policies
If you are not completely satisfied with any of Maui
Dive Shop products, you may return them for an exchange or refund within 30 days of purchase. Any product
returned for exchange or refund must be in new condition.
Shipping charges for items returned will not be refunded.
In order for us to provide you with fast efficient service, please follow these instructions when
returning merchandise.
1. Upon receipt of your items, immediately check to verify that the size is correct.
2. Inspect the merchandise to be sure that the design, style or color is correct.
3. Any merchandise that is returned must be in new condition. Do not wear, wash or iron the product.
4. Please be sure to retain the invoice.
Return Process:
1. Contact Maui Dive Shop via e-mail or telephone (contact information provided below).
2. Please provide us with the reason for returning the item(s) and specify if you would like to exchange
the item or if you are requesting a refund.
3. If you are exchanging an item, please include proper information on the replacement item.
4. Ship the item to the address listed in the contact information and please be sure to enclose the
invoice. The invoice is needed to complete the exchange or refund. Maui Dive Shop will not pay shipping
charges. We strongly recommend that you use a delivery confirmation when you ship otherwise we cannot
guarantee receipt of your merchandise.
5. Maui Dive Shop will notify you when we have received the product and process your request quickly,
based on receipt of all documents requested in our guidelines.
